Kaleidoscope Care is a model of care designed to assist caregivers as they determine:
Caregivers use an assessment tool to gauge the range of each person’s ability. Then they use the Kaleidoscope color-coded ranges of function to determine the approaches that are likely to work best for the individual. Kaleidoscope integrates the resident’s cognitive level into all aspects of care – and this tool is used by all campus staff – Caregivers, Housekeepers, Dietary staff, Physical Plant, Activities and Social Services, as well as Administration, Volunteers and Families.
The four color-coded ranges of cognitive function include: Emerald (cognitively intact), Topaz (mild cognitive impairment), Ruby (moderate cognitive impairment), and Sapphire (severe cognitive impairment).
Trilogy caregivers receive specialized training in the Kaleidoscope Model of care.
